summary

A few days ago, when the unit organized a physical examination, a colleague of the unit found out the atrial septal defect. Later, he went to the hospital and had a detailed examination. Now let's share what we should pay attention to.

What should we pay attention to in atrial septal defect

First, the primary or secondary atrial septal defect can be distinguished, because the repair methods of the two defects are different, and the secondary atrial septal defect can be directly sutured; The primary foramen defect must be repaired with patch, and there is a risk of atrioventricular node (between atrioventricular ring and coronary sinus) and conduction bundle (at the upper edge of coronary sinus) damage, resulting in Ⅲ degree atrioventricular block. Some patients may be combined with anterior mitral valve cleft, such as improper suture, prone to mitral stenosis or insufficiency.

Second, pay attention to the repair methods of sinus type and inferior vena cava type atrial defect. Patch should be used to repair sinus type atrial defect. If it may cause stenosis of superior vena cava, a piece of pericardial piece or polyester cloth can be used to sew at the junction of atrium and superior vena cava to widen it, so as to expand the superior vena cava. If we don't pay attention to the identification, it is easy to mistake the inferior vena cava flap as the inferior edge of the defect. After suture, the inferior vena cava will be separated and led into the left atrium, resulting in cyanosis.

Third, pay attention to correct the combined malformation at the same time. The most common combined malformation is the ectopic connection of the right pulmonary vein. When repairing atrial septal defect, the ectopic pulmonary vein should be separated into the left atrium without affecting the right pulmonary vein orifice, so as to avoid stenosis.

matters needing attention

Patients with atrial septal defect must have a good living habits in daily life, and give up bad living habits, including pregnant women and their spouses, such as smoking, drinking, etc.; actively treat diseases affecting fetal development before pregnancy, such as diabetes, lupus erythematosus, anemia, etc.
